





This form is a US Post Office Department printed form. I have seen variations used between 1932 to 1966, and it was likely used outside those dates.
Here is the version from 1932, the Form 2966 must more legible.


From a practical viewpoint, I suppose that the tag could be used with some other transportation service, but I have not personally seen such usage. I would assume that Railway Express had a similar form that they used for their international shipments and customs declarations.

Yes, but the form is specifically addressed to "PO Carrot Creek, Alberta, Canada"
A bit simpler than today's form. It is a government form number but it does not appear to be specifically intended for Post Office use. Does anyone know if this form was also used on other cross border shipments (thinking Railway Express, etc).
