Here's a close-up of the perfs between the second and third stamps:

So all you world-wide collectors, do you have any examples of this anomaly? Is there a philatelic term to describe it?

Even better, does anyone know HOW this could occur?

That's interesting and does make sense. Looking at the way those full perfs were done, the operator actually got the alignment right on the mis-perf and should have left it alone.

This rather grotty specimen arrived today, which had I spotted on Hipstamp. I've seen examples of double perforating before but not a 'half double'. It appears to be progressive too, in that each row advances to the right by a fraction:
