The later US issue has a separate major Scott catalog number mainly because the denomination has changed from the international 84¢ rate to the domestic 42¢ rate, not just because of the change in format.
Canada issue came in 2 formats:
-- pane of 16 (8 pairs), singles are Scott #2204 and #2205, the pair is #2205a, the pair in reverse order with gutter between is #2205i
-- souvenir sheet of 2, Scott #2205b

The Finland souvenir sheet of 2 is Scott #1280(24Jan2007).
The Sweden souvenir sheet of 2 is Scott #2552(25Jan2007).

The U.S. issued the stamps separately in panes as 4203-04. Canada issued them separately in panes as 2205a. I don't know of any others so far, but Denmark and Greenland seem to be good candidates if the Scott numbering system is consistent. (Stop laughing Michael!).
You mentioned Canada 2205i. That looks to be a gutter between SS panes, not 2205a panes.

"No that is the gutter between the stamp pairs within the 2204-2205 pane. The 8 pairs are arranged in 2 groups of 4 pairs, with a gutter between the 2 groups. So you could separate the pane into horizontal strips of 2205+gutter+2204. That is what Unitrade calls #2205i."
My bad, Lars. I was too lazy to post a pic of the full pane of 16 stamps showing the internal gutter. That would have made it more clear.
I still haven't gone home yet. I'll check the Facit then.
It looks like the only unsettled question is whether Denmark and/or Iceland issued their stamps in any format other than SS.

It does appear to indicate that the other items on your list were only produced in souvenir sheet format (only US and Canada released miniature pane versions).

I'm trying to find Scott numbers for the 7 other issues that were joint with US 4123 - International Polar Year 2007-2008. The hard part is that US also issued these same stamps later in the year in panes of 20 as 4203-04, so I'm afraid the numbers I have for Canada, Denmark, and Greenland are for similar pane issues and not the Souvenir Sheet.

I have 2204-05, but I wonder if that is for a separate pane issue.
